TEAM REGISTRATION
Registration Fee: $1,200 per team, due by May 1, 2010.
Teams have the option of paying a $300 deposit at time of registration with the remainder due on/before May 1, 2010.
Registration for this event is now closed.
CANCELLATION POLICY
If the full $1,200 is paid at the time of registration, 75% is refundable until May 1, 2010.
If $300 is paid at time of registration, the initial $300 deposit is non-refundable.
U13 PARTICIPANT INFORMATION AND AGE DEFINITION
All current US Lacrosse members who meet the age requirements and have never played in high school are encouraged to contact their local chapter regarding the U-13 National Youth Festival. Players may not be 13 years of age on or before December 31, 2009 to be eligible for the 2010 US Lacrosse U-13 Youth Festival. Accordingly, players that turn 13 after December 31, 2009 are eligible. Regardless of age, no player who has competed at the high school level is eligible to participate. 9th graders are not allowed to participate no matter what the situation.
Reminder: A player may only participate once at this age level.

Roster Size: 22 Players per team MAX - 2 coaches and 4 chaperones (coaches can be chaperones)
Festival Rules: We will utilize the posted Youth Rules as listed in the respective rule books below:

KEEP TRACK OF YOUR TEAM
US Lacrosse is using Youth Leagues USA (YLUSA) to provide all online forms for the U13 Lax Fest.
How does this work? As players and coaches complete the required forms, their information is captured in your team’s database and creates a team roster. As a team coordinator, you will be able access your team’s database and keep track of your players and coaches forms and status with US Lacrosse (i.e.- membership and proof of age received). Team coordinators must complete a team commitment form to receive a user name and password. Once a username and password is received, coordinators will be able to:
• View real time membership status and form and document completion;
• Send email blasts to coaches, chaperones, players, and parents;
• Update team information such as team colors, jersey numbers, etc.
• Access team medical emergency information.

YOUR TEAM MUST BE OFFICIALLY REGISTERED WITH US LACROSSE BEFORE COMPLETING THE FOLLOWING FORMS. The TEAM COMMITMENT FORM IS NOT THE OFFICIAL REGISTRATION FORM. You must contact the Events Department at events@uslacrosse.org to see if there is room available for your team.

BACKGROUND CHECKS FOR COACHES AND CHAPERONES ARE MANDATORY
Thank you for taking on the role as coach for your youth team this year. It’s definitely a lot of work but also very rewarding! This is the first year for required background checks for both Coaches and Chaperones. Below is a list of accepted background checks. Please read the following carefully.
• If you are a US Lacrosse Level 1 Certified coach, then you have already had the background check competed and US Lacrosse has this on file. There is no need to complete another one.
• If you have had a background check completed through your current employer, we will accept a copy or a written documentation from your employer that you have passed.
• If you have not had a background check completed, click here for directions to do so through a US Lacrosse partner, NCSI. The process will take at least 10 BUSINESS DAYS to complete and the cost is $22.50 per person.(Additional fees maybe assessed if you live in the following states: (CA, LA, MN, MS, NV, NY, SD, TN, VT).
RESULTS must be received by US Lacrosse no later than Monday, JUNE 7.
