2011 Registration Fees*
• Regular Registration (through Jan. 9): $100.00
• On-Site Registration (Jan 20-22): $150.00

Optional
• CPR & AED Certification Class (Friday, Jan. 21 • 8-11 am): $40.00
(Sign-up through the main convention registration link)

Group Registration
Bring your colleagues along in order to receive a special discount: Ten (10) or more US Lacrosse Members who register for the convention as a group will each get $10 off the registration price.
► Please designate one person to submit this roster and they will receive an invoice for the total payment.

* Payments for Purchase Order registrations must be RECEIVED by January 7, 2011 to complete your order. On-site registration fees will apply upon failure to complete your order. To register via Purchase Order, please contact us at convention@uslacrosse.org.

REGISTRATION IS FINAL. NO REFUNDS OR TRANSFERS WILL BE ISSUED.  
Persons must be at least 18 years of age and current US Lacrosse members in order to register for the convention.
