The fourth annual US Lacrosse U15 National Championship will feature as many as 32 of the best boys’ teams and 32 of the best girls’ teams from around the country. Teams may qualify in three different ways:

(1) Returning Top 4 Teams and Sportsmanship Winner
The 2011 national champions and second-, third- and fourth-place finishers, along with the sportsmanship award winners, receive automatic bids to the 2012 national championships and will pay a registration fee of $1,500.

Boys:
For the Love of the Game (N.Y.) (champion), Long Island Jesters (N.Y.), Long Island Express (N.Y.) and NorCal Talons
Sportsmanship Award: Jacksonville Lacrosse Club (Fla.)

Girls:
Long Island Yellow Jackets (N.Y.) (champion), Stars Gold (Va.), Top of the Bay (Md.) and Long Island Top Guns (N.Y.)
Sportsmanship Award: Desert Heat (Ariz.)

(2) Winning or Placing in the Top 4 at a 2012 Regional Championship
US Lacrosse will host three regional championships in June. Each regional champion and second-, third- and fourth-place finisher will earn an automatic bid to the national championship with free or discounted registration. That’s up to 12 slots total for each national championship, and this option represents the only guaranteed way for teams to earn bids to the national championships in Orlando.

(3) Application Process for At-Large Bids
At-large bids for 15 boys’ teams and 15 girls’ teams will be awarded via an application process after teams completed a Team Application Form.

Background Checks
Background checks are required for all coaches and chaperones. This is MANDATORY.

Below is a list of accepted background checks. Please read the following carefully.

If you are a coach certified through Level 1 of the US Lacrosse Coaching Education Program (CEP), then you have already had the background check completed and US Lacrosse has this on file. There is no need to complete another one.

US Lacrosse will no longer accept a background check completed through your current employer. All background checks must go through NCSI.

If you have not had a background check completed, click here for directions to do so through a US Lacrosse partner, NCSI. The process will take at least ten (10) BUSINESS DAYS to complete and the cost is $22.50 per person. Additional fees may apply if you live in the following states: CA, LA, MN, MS, NV, NY, SD, TN, or VT.

Background check results must be received by US Lacrosse no later than May 6, 2012, so please plan accordingly.


Championship Roster Size
24 players per team MAX – four coaches and four chaperones (coaches may double as chaperones).
Note: Teams that participate in a U15 regional championship will have an opportunity to update their final roster with up to six (6) players.

Championship Format
Three-day event, with pool and bracket play.

Championship Registration Fee
$1,500 per team, via credit card. A $500 deposit, via credit card, is due at the time of application for an at-large bid, as per option 3 above.

Cancellation Policy: If fee is paid at the time of registration and your team is accepted, 75% is refundable until March 15, 2012. After March 15, 2012, the registration fee is non-refundable.
