Participating Teams Checklist

The following forms are to be completed by June 6, 2005 and mailed to:

• Youth Festival Housing, Game, Facility Fees (PDF)

1. One Check made payable to US Lacrosse to cover Games Management. This fee is due for each Youth player participating in the 2005 US Lacrosse U-13 Youth Festival. 24 players X $50.00 = $1200.00 per team

2. Teams are strongly encouraged to stay on campus as both a financial decision and to allow the players the full campus experience.  Please note there will be an additional facility usage fee for all teams staying off-campus ($25 per player). 24 players X $25 = $600.00 per team

3. One check, made payable to US Lacrosse, to cover housing expenses for all team members (player, coach/ chaperones) staying at Binghamton University. (Please send one check only, US Lacrosse is not equipped to handle 26 individual payments.) 28 participants X $135.00 = $3780.00 per team

• Completed Official Roster (Word Document)
Please include each person’s US Lacrosse membership number. Youth Festival team roster forms are attached and must be submitted electronically to assure accuracy.
Roster MUST include coach/chaperones. All rostered individuals must be members of US Lacrosse. Rosters must be 100% complete. Incomplete rosters will need clarification and will require extra processing time which will delay your team at registration.

• Medical Waiver/History Form (PDF)
For each rostered player (Must be signed by parent/guardian)

• Youth Festival Conduct Pledge (PDF)
For each rostered player and coach/chaperone (Must be signed by each participant)

For each coach/chaperone (Must be signed by coach/chaperone)

• Coach/Chaperone Disclosure (PDF) & U-13 Youth Regional Festival Rules and Regulations (PDF)

NOTE: If the forms and payment listed above are not received at US Lacrosse by June 6, 2005 in their entirety, US Lacrosse reserves the right to reassign your rooms to another team. Please do not send a partial package of team information. Incomplete packages will need clarification and will require extra processing time which will delay your team at registration.

**Housing Forms must be returned to US Lacrosse by June 6, 2005 **

Due to the layout of dorms at Binghamton University, we cannot guarantee that every team will receive rooms on the same floor. Rooming assignments will be done on a first-come, first-served basis. Therefore, we encourage teams to send their housing assignments as soon as possible.
