IMPORTANT NOTES
• All teams staying outside of the Walt Disney World® Resort are required to pay a $600 facility usage fee. The fee must be submitted to US Lacrosse with their completed Participant Information Packets by May 15, 2006.

• Disney’s Wide World of Sports® Complex does require a spectator admission fee to enter all events within the sports complex. However, for the 2006 US Lacrosse U-15 National Youth Festival, US Lacrosse will be absorbing the cost of these fees, allowing spectators to enter the event at no charge.

US Lacrosse Payment and Cancellation Policies

Reservation Policy
All reservations must be made prior to May 19, 2006 and availability is on a first-come, first-serve basis. After May 19, 2006 all reservations will be subject to availability.

Package Payment Policy
Reservations made prior to April 24, 2006 require a $25.00 per person non-refundable deposit at the time of booking. On April 24, 2006, an additional non-refundable deposit of $50.00 per person is due and payable. Full payment is due by June 2, 2006.

Reservations made on or after April 24, 2006 require a $75.00 per person non-refundable deposit at the time of booking. Full payment is due by June 2, 2006.

Anthony Travel will charge all payments due to the credit card on file for the reservation. Should the credit card decline, Anthony Travel will attempt to contact the guest via email and phone. Should payments not be made in accordance with the dates presented above, Anthony Travel shall cancel reservation and apply appropriate cancellation penalties.

All charges made on this reservation will appear on your credit card statement as a charge from Anthony Travel.  If for any reason you or someone booked under your reservation number disputes a valid charge from Anthony Travel with their credit card company, a $25.00 fee will be applied to your account and charged to the credit card on file.

Package Cancellation Policy
A $25.00 per package cancellation fee applies to cancellations made between time of booking and April 24, 2006. A $75.00 per package cancellation fee applies to cancellations made between April 24, 2006 and May 19, 2006. Package cancellations after May 19, 2006 are fully non-refundable. All cancellations must be made in writing to Anthony Travel.

Room Only Policies

Room Only Payment
Reservations made prior to April 24, 2006 require a non-refundable deposit of one night’s room and tax at the time of booking. Full payment is due by June 2, 2006.

Reservations made on or after April 24, 2006 require full payment at the time of booking.
