Registration Process Registration for the US Lacrosse U-15 National Championships consists of four (4) steps: 1 Complete the online registration and submit the $1,200 team registration fee. 2 Submit a full roster with proof of age for each player (see Tournament Rules below for details) 3 US Lacrosse membership and medical waiver must be completed by parents or guardian. 4 Onsite registration where each player must provide hard copy of Proof of Age
Team Forms All Primary Team Contacts must complete a Team Commitment Form before players, coaches and chaperones can complete other required forms. Once the Team Commitment form is complete, players, coaches and chaperones will be able to select the appropriate team and complete their forms. Due dates for required paperwork: May 15 - Team Commitment Form June 15 - Coach/ Chaperone Form June 15 - Player Medical Waiver June 15 - Proof of Age
Note: Please make sure you select the correct event and division (boys/girls) before completing the correct form
Refund Policy Our refund policy is designed to minimize the impact on the event and its participating teams in case a team already selected for the event drops out. If a team drops out of the event before February 10, 2009, 50% ($600) of the team's registration fee will be refunded. If the team drops out after February 10, 2009, the registration fee is non-refundable. Teams that are not selected from the waiting list by March 1 (the official close of registration date) will receive a full refund of their team registration fee.
Tournament Rules 1 All players and coaches must be current US Lacrosse members during the event. 2 All game competition played in accordance to the set Youth Rules as contained within the 2009 NFHS Boys High School Rule Book and 2009 US Lacrosse Womens Rule Book. 3 All players meet the age requirements and provide proper proof of age documents (i.e.- birth certificate, passport, or picture ID with valid date of birth). 4 All teams must book their hotel accommodations through the official travel and lodging partner: Outerzone Sports
Age Definition
Player may not be 15 years of age on or before December 31, 2008 to be eligible for the 2009 US Lacrosse U15 National Championship. Accordingly, players that turn 15 after December 31, 2008 are eligible.
| US Lacrosse recognizes that this age definition does not match what is currently in the 2008 and 2009 NHFS and US Lacrosse rule books. The disqualifier for high school competition used at the US Lacrosse U-15 Youth Festival was purposely omitted from the participation requirements for this event to allow for broader participation. The US Lacrosse Youth Council Boys and Girls Youth Rules Sub-Committees annually review recommended youth rule changes and this rule is under consideration for addition into the 2010 rule books for some events run by US Lacrosse. |
