Group Membership Registration

Welcome. Please follow these steps to register your program.

1. Set-up your group account.
Complete the online group set-up form to establish your group account.

2. Gain access to your account.
Complete the Data Access Agreement to gain access to your group account. Here you can manage your players’ and coaches’ membership numbers and expiration dates.

3. Register your members.
There are three different options to register your members

• Option #1
As Program Administrator, you will collect the US Lacrosse registration form and individual payment for each member. Enter the data into the US Lacrosse Excel template and submit the Excel template via e-mail. Submit one payment for all players and coaches.

• Option #2
Members can go directly to www.uslacrosse.org and "Join Now." During the registration process they will select your group as their primary affiliation.

• Option #3
As Program Administrator, you will collect the US Lacrosse registration form and individual payment for each member. Submit one payment for all players and coaches with paper forms to US Lacrosse.

4. Obtain your liability insurance.
Request your certificate of liability insurance online at www.Bollingerlax.com.
