DIRECTIONS: All interested organizations must submit a Proposal Form in order to be considered for a grant or service. This form ensures your organization has the opportunity to apply for all the resources you are qualified to receive. One form will be completed for all your needs as an organization. Forms must be filled out accurately and with as much detail as possible; while approval is not guaranteed, the more detail you provide, the greater your chance is of being approved. This is your opportunity to present your needs as an organization to US Lacrosse. Please do not submit multiple forms on behalf of an organization. Only one Proposal Form per organization will be accepted and reviewed. All fields are required. You can type more than the box allows; a scroll will appear on the right side of each box.
