Thank you for you for inquiring on behalf of your organization to be considered for a grant or service from US Lacrosse!
DIRECTIONS: All interested organizations must submit a Proposal Form in order to be considered for a grant or service. This form ensures your organization has the opportunity to apply for all the resources you are qualified to receive. One form will be completed for all your needs as an organization. Forms must be filled out accurately and with as much detail as possible; while approval is not guaranteed, the more detail you provide, the greater your chance is of being approved. This is your opportunity to present your needs as an organization to US Lacrosse. Please do not submit multiple forms on behalf of an organization. Only one Proposal Form per organization will be accepted and reviewed. All fields are required. You can type more than the box allows; a scroll will appear on the right side of each box. Complete the form below in its entirety, then click SUBMIT.
SUGGESTIONS: You will not have the option to save your progress and resume this form. Please plan accordingly and allow yourself adequate time to answer all questions. We suggest printing this form to prepare your answers before completing the online proposal.
